Children’s Home Society of Florida’s Transitional Living Program (TLP) assists young adults, ages 18-23, who have aged out of the foster care system with earning higher education, finding jobs, stable housing, financial support, and other life necessities. TLP counselors help identify initial and long-term goals and objectives based on education, employment, and income/budget needs in order for the youth’s success and development of postive life skills. Upon completion of the Youth Transition Center, the program will provide up to 6 months of follow-up services by a TLP counselor to ensure that they are able to maintain stability and independence.

Skills Training and Job Placement
To break the generational cycle of child abuse, individuals must have the skills necessary to obtain self-sufficiency. Youth in the Transition Center, and non-resident youth through the Transition Living Program, will be connected with Workforce Solutions to prepare them for workforce readiness through the Work Certified Program and Passport Program. Workforce Solutions will provide on-site employment counseling and connect the youth to Indian River State College and other institutions of higher learning for post-secondary education and vocational training to prepare them for careers with the greatest potential for placement in the workplace.

Mentoring
Adult role models are key influences for youth maturing into successful adulthood. The Teen Life Choices Program helps youth develop social skills through individualized mentoring relationships, Career Days, conducting tours of job sites, and teaching independent living, social and vocational skills.

Health Care
Access to health services is key to the emotional and financial stability of former foster youth. We provide counseling services onsite through our residential programs and will coordinate services from various community providers to reaffirm Transition Center youth’s progress toward self-sufficiency.
